Accessing Transitional Housing Funds

 

  • Attend (or review) the required web-based training;
  • Sign and submit Agreement & Certification Form (required to access funding);
  • Complete and submits ACH Vendor Enrollment Form;
  • Certify that survivor is eligible for the transitional housing program using Initial Amount Request Application;
  • Work with survivor to identify what individual transitional housing needs are;
  • Complete and submit initial paperwork requesting access to funds on behalf of the survivor (required);
  • Idaho Coalition approves or denies request;
  • If request is approved, program or community partner makes payment for housing needs identified (and approved) in the Initial Amount Request Application;
  • Program or community partner submits monthly invoicing and supporting documentation to Idaho Coalition for approved transitional housing reimbursements (required);
  • Idaho Coalition approves monthly invoicing as submitted and makes reimbursement payment to program or community partner;
  • Program or community partner submits Exit form when survivor exits or is terminated from transitional housing program (partner must contact the Idaho Coalition prior to terminating a survivor from the program);
  • Program or community organization submits Follow-Up Support Services Verification form once follow-up support services have been provided for the required period of time.
